Perfect Smile Spa Terms and Conditions
1. Appointments and Cancellations:
- Patients are required to schedule appointments in advance.
- Cancellations must be made at least 48 hours in advance.
- Failure to cancel within the specified timeframe may result in a cancellation fee.
Consultation booking fee
A £20 refundable booking fee is payable to secure a consultation. If you attend your consultation, the fee will be refunded in full or, if you choose to go ahead with treatment, deducted from your treatment cost. If you do not attend your consultation, the booking fee is non-refundable.
2. Registration and Patient Information:
- Patients must provide accurate and up-to-date personal information during registration.
- Any changes in contact details or health information should be communicated promptly.
3. Fees and Payments:
- Fees for dental services will be provided to patients in advance.
- Payment is due in advance of service unless alternative arrangements have been made.
- Late payment may incur interest charges.
Consultation booking fees are covered under Section 1.
4. Dental Treatment:
- Patients will be informed about proposed treatments, associated risks, and costs.
- Informed consent will be obtained before commencing any dental procedure.
5. Emergency Procedures:
- Procedures for handling dental emergencies will be explained to patients.
- Emergency contact information will be provided to patients.
6. Confidentiality:
- Patient information will be kept confidential in accordance with data protection laws.
- Exceptions may include situations where disclosure is required by law.
7. Patient Conduct:
- Patients are expected to treat staff and fellow patients with respect.
- Any disruptive or inappropriate behaviour may result in termination of services.
8. Complaints Procedure:
- A clear procedure for handling patient complaints will be followed.
- Patients will be provided with information on how to submit complaints.
9. Termination of Services:
- The practice reserves the right to terminate services for any reason.
- Patients will be informed in writing if services are terminated.
10. Governing Law:
- Any dispute arising under these terms and conditions shall be subject to the exclusive jurisdiction of the courts of England and Wales.
11. Changes to Terms and Conditions::
- The practice reserves the right to update these terms and conditions.
- Patients will be notified of any changes.
